Sanjeevani (संजीवनी) – Community Health and Wellbeing Project

Role Summary
The Nurses will be responsible for delivering community-based primary health services under the Sanjeevani Project at the Health Sub-Centre (HSC) level. The role includes identification of communicable and non-communicable diseases (NCDs), referral of cases to appropriate Government health facilities, community health education, and coordination with frontline health workers to strengthen access to public health services.
Key Responsibilities
- Identify, screen, and document cases of communicable and non-communicable diseases, including maternal and child health conditions
- Facilitate timely referral of identified cases to Health Sub-Centres, Primary Health Centres (PHCs), Community Health Centres (CHCs), and District Hospitals, as required
- Coordinate closely with ASHAs, Anganwadi Workers (AWWs), and other frontline health staff for outreach, follow-up, and continuity of care
- Conduct community awareness and health education sessions on disease prevention, nutrition, hygiene, sanitation, and healthy lifestyles
- Maintain patient records, referral registers, and activity reports
- Ensure adherence to Government health protocols and project guidelines
Qualifications & Experience
- ANM / GNM qualification from a recognised institution (registration with State Nursing Council preferred)
- Minimum 1–3 years of experience in community health, primary healthcare, or public health programmes
- Experience working in rural, tribal, or underserved areas will be an added advantage
Key Skills & Competencies
- Sound knowledge of primary healthcare, communicable diseases, and NCDs
- Strong interpersonal and community engagement skills
- Ability to work effectively with Government health systems and frontline workers
- Willingness to work in tribal and remote areas
- Proficiency in local languages preferred
- Candidates from Jharkhand will be given preference
